Enrolling as a patient at Grace C Mae Advocate Center, Inc. (GCMAC) is easy! Anyone can refer a patient, and self-referrals are welcomed. To schedule an appointment, call (319) 361-6529 or email firstname.lastname@example.org. An intake specialist will ask for name, address, phone number, date of birth, and insurance information. Patients are expected to contact their insurance company for any required preauthorizations, if needed. A photo ID and a copy of the insurance card(s) are required at the first appointment. The initial assessment usually lasts about an hour. During that appointment, a complete social history is obtained, the paperwork is completed, and the beginning of a treatment plan is developed. In some cases, a safety plan is necessary and it will be developed within the first three sessions. Most patients can expect therapy to last about 12-16 weeks, depending on the goals and condition of the patient. Every treatment plan is individualized for each patient, and no two patients will have the same experience during the course of their treatment. Sometimes, patients will choose to change their goals or extend their treatment by adding new goals to work on. It is not too unusual for some patients to choose to remain in treatment for a year or more, depending on how many goals they want to achieve.
Payment is expected at time of services unless prior arrangements have been made, including co-pays and deductibles. We are happy to bill insurance on behalf of the patient; however, the patient’s share is expected to be paid at the time of service.
